In-Home Outpatient Physical Therapy
Comber Physical Therapy and Fusion Chiropractic is introducing In-Home Outpatient Physical Therapy to help address the growing need for clinical options in treating high-risk patients during the COVID-19 pandemic. Our in-home therapy is designed for those who may be at higher risk to exposure of viral outbreaks. This is not a home health service for homebound patients.
In-home outpatient physical therapy is the same level of care as the physical therapy offered in our clinics, but presents the convenience of concierge care in your home.
What you can expect:
Your physician will send a referral to our referral coordinator. Our referral coordinator will contact you to create an electronic medical record and schedule your first appointment.
You will receive a welcome email with patient paperwork to be completed and returned to our referral coordinator by FAX (757-903-4231) or email (firstname.lastname@example.org). Your therapist can return the original documents to the clinic after your first appointment.
Our physical therapist, Karen McPadden – MPT, will arrive at your home at the scheduled time. She will be driving our Comber car and present her name badge so you can verify her identity.
The physical therapist will follow all CDC guidelines for limiting exposure to both viruses and bacteria. You can expect her to wear a mask throughout the visit.
If you are a new patient, an evaluation will be performed. This could take up to an hour. If you are an existing patient, your therapist will perform follow-up care which could take up to 45 minutes.
Your therapist will create a Plan of Care based upon your medical history and therapy goals, including a Home Exercise Plan to utilize during the time between your in-home therapy visits.
Your therapist will schedule future appointments in conjunction with our medical reception team prior to leaving your home.
Should you become ill and unable to keep your scheduled appointments, please cancel at least 24 hours in advance to allow our therapist to adjust her schedule and see other patients. Failure to do so may result in a $50 late cancel fee to cover our therapist’s travel time. If you have a fever, cough, or shortness of breath, please consider the safety of our therapists and reschedule your appointment.
